Have you ever had a deep pain in your buttocks? or perhaps you had a niggle and felt tingling shoot down your leg from your glutes? Deep Gluteal Pain Syndrome is a collective term used to describe numbness and deep pain felt in the Glute muscles!


Firstly, Let's Dive into the Complex Anatomy of the Glutes & Why are they Important?

Did you know that the Glute muscles are the largest muscle in the body? And for good reason! The Gluteus muscles (medius, minimus, maximus namely) help stabilise and anchor the upper body & pelvis, aiding in locomotion and propeling the body forward throughout gait!

The gluteus maximus is the prime mover of abduction, it helps to support and stabilise proper leg alignment by controlling the internal rotation of the thigh, it acts as a fulcrum! The gluteus medius helps with internal rotation and abduction whilst the gluteus minimus stabilises the pelvis during single leg movements.

The small muscles of the piformis overlie the sciatic nerve, when this muscle is impinged and the surrounding muscles are tight, the nerve becomes irritated which can produce numbness and tingling!


What are the Symptoms of Deep Gluteal Pain Syndrome?

The Typical signs and symptoms of DGPS include but are not limited to:

  • Buttock and pain at the trochanter (where your hip bone meets your leg bone!)

  • Pain in the buttocks with prolonged sitting

  • Pain when walking in the glutes

  • Sensory changes such as tingling and numbness that may extend into the legs

  • Night pain

What are the Treatment Options Available?

Treatment and management of gluteal pain is varied and often includes more than one modality. Depending on how the case presents, treatment may include a combination of:
